What is a DLM1 Medical?
A DLM1 medical report is normally required when applying for or renewing a Northern Ireland DVA Group 2 lorry or bus licence. A doctor completes the form, which you submit to DVA with your application or renewal documents.

What is a DLM1 medical?
A DLM1 medical is the Northern Ireland medical report used for Group 2 lorry and bus licence applications or renewals. It helps the DVA assess whether you meet the required medical standards for vocational driving.
Is a DLM1 the same as a D4 medical?
They are similar types of driver medical forms, but DLM1 is the form used in Northern Ireland for DVA Group 2 lorry and bus licensing. D4 is the term more commonly used in Great Britain. If you are based in Northern Ireland, DLM1 is usually the form you need.
Who needs a DLM1 medical?
You may need a DLM1 medical if you are applying for or renewing a lorry, bus, HGV, LGV or PCV licence entitlement in Northern Ireland.
How long does the appointment take?
Most appointments take around 15 minutes, provided you bring the correct paperwork and any relevant medical information with you.
What should I bring to my DLM1 appointment?
Please bring your DLM1 form, photo ID, glasses or contact lenses if you use them, your most recent optician prescription if available, and a list of any medication you take. If you have relevant medical conditions, it is helpful to bring a GP summary or recent hospital letters.
Will the form be completed during the appointment?
Yes. The aim is to complete the relevant medical sections of your DLM1 form during your appointment, so you can send it to the DVA afterwards.
