What is an HGV or LGV Medical?
An HGV or LGV medical is a health assessment for drivers applying for a lorry licence or renewing their entitlement. It checks that you meet the Group 2 medical standards for driving heavy goods vehicles.
The assessment is carried out by a GMC-registered doctor and includes a review of your medical history, blood pressure and eyesight checks. The doctor completes the relevant DLM1 medical report for Northern Ireland DVA applications, or D4 report for Great Britain DVLA applications.

What is a DLM1 medical?
A DLM1 medical is the assessment used to complete the medical report for a Northern Ireland Group 2 lorry or bus licence. The doctor records your medical history and examination findings so DVA can assess your fitness to drive.
Is a DLM1 the same as a D4 medical?
Both involve a driver medical, but they use different forms. DLM1 is used for Northern Ireland licences issued by DVA, while D4 is used for licences issued by DVLA in Great Britain.
Who needs a DLM1 medical?
You may need a DLM1 medical when applying for or renewing your Northern Ireland lorry licence. Your DVA application or renewal paperwork will confirm whether a medical report is required.
How long does the appointment take?
Most appointments take around 15 minutes, provided you bring the correct paperwork and any relevant medical information with you.
What should I bring to my DLM1 appointment?
Please bring your DLM1 form, photo ID, glasses or contact lenses if you use them, your most recent optician prescription if available, and a list of any medication you take. If you have relevant medical conditions, it is helpful to bring a GP summary or recent hospital letters.
Will the form be completed during the appointment?
In most cases, yes. The doctor will complete the medical sections of your form during your appointment. If further information is needed, we’ll explain the next steps.

Do I send the DLM1 form to the DVA myself?
